Once homeless Jimmy has calmed down, he's allowed back into the hospital to have his hand treated, which was injured when a gang of youths smashed his tent up and stamped on his hand. Plastics Doctor Christie Brennan, and Registrar Dr. Andrew Rennie treat Jimmy, but are forced with a difficult decision. As his wound can't be treated on the night shift, do they admit him, or send him back on the streets?

A 17-year-old has arrived at London's North Middlesex University Hospital after falling backwards and hitting his head. The deep cut is gnarly but a near loss of consciousness is the main concern. For the patient however, the main concern is avoiding shaving his hair.

He is known to staff at the hospital already, as a frequent visitor. After falling and rolling her ankle, 39-year-old Natasha is being seen by Dr Chris in North London. As it's been 6 hours and she still can't bear weight on it. the concern is that Natasha has a fracture.

40 miles south, the nightshift in Tunbridge Wells Hospital is underway with no shortage of dislocations for Dr Ahmed. Two friends arrive in resus who have bizarrely dislocated each other's shoulders during a friendly wrestling match. It's a painful procedure attempting to manoeuvre the shoulder back into place, and even with Dr Ahmed using all his strength, it won't budge. Both shoulders prove tricky, with one dislodging again!

150 miles south in North Middlesex University Hospital, another cyclist has been hit by a car. Having just delivered a takeaway, this patient was taken out on his bike by an unknown driver. Dr Kefe is worried about multiple injuries with this mechanism of injury, and orders and array of x-rays. As the ambulance service is overrun, 36-year-old Terry took a taxi to A+E after dislocating his knee while shopping.

The patient is agitated but Dr Alex's primary concern is spinal damage and brain injury. They transfer the patient to resus just in time to intercept a seizure. Time is critical but this patient's behaviour is getting in the way of a life-saving brain scan.

A&E in London's North Middlesex University Hospital is heating up thanks to an aggressive male who claims he has been attacked. Language is a barrier but by a stroke of luck, Advanced Clinical Practitioner Albena translates. The patient is incoherent, threatening, and is a risk to others, so security stand close until his suspected psychosis can be managed.

Police escort a violent man injured during his arrest to A&E. As he resists leaving the van, security is called to contain him. Eventually brought inside, his aggression forces staff to isolate him until he calms down.

Tensions run high as a man with potentially severed fingers becomes abusive while waiting his turn. He storms outside to smoke crack, forcing security to intervene.
