This episode featured six celebrity contestants competing for charity. The winner would receive $50,000 for their charity, and the others would receive 25,000 dollars for their charities.

Contestants retrieve a flag from the wing of a biplane at 4,000 feet, pluck plums from a water tank full of snakes and drive a car off a parking structure.

Bull Riding Contestants would each have to ride a 2,000-pound bull. The two men and the two women who stayed on their bulls the longest would advance to the next round. The others would be eliminated.

Contestants must jump between speeding boats, retrieve weights from a tank of rotting squid and climb a flag pole 20 stories up.

Contestants must traverse a cargo net underneath a helicopter, eat pig rectums and walk an obstacle-strewn ledge 11 stories up.

Playboy Playmates walk a tightrope 10 stories up, eat strawberries from an insect-filled box and retrieve glow sticks from freezing water.

Contestants climb down the side of a blimp, bob for chicken feet in a vat of wax worms and flip a speeding car.

Helicopter Climb Each player would hang from the bottom rung of that ladder and as the helicopter spun above the cold Pacific Ocean, they would have to climb up and into the "bird". The four players with the best times would advance.

Contestants must traverse a narrow beam, place their heads in a rat-filled box and cross between two moving trucks.

Contestant bobs in tank of cow blood trying to retrieve as many dive rings in two minutes. They shoot blow darts to determine the number of tomato hornworms that they will eat, and crawl through dark tunnels and retrieves 3 dead skunks in as fast a time as possible.

All Gross

Contestants must walk around a 12 inch perimeter while an apparatus which is turning raised in the air, fastest time without falling wins. After that its the Eel tank and Heli Rodeo.

All Female Fear Factor

Contestants must jump between two moving flatbed trucks, lie in a coffin filled with worms, and be tumbled in a giant cylinder over water.

For the first stunt, contestants ride a raft behind a speedboat. They climb a rope hanging from a helicopter and release 6 flags before dropping 20 feet into the water. If contestants completed this stunt, they would win $1,000.

Contestants try to stop a kart on an elevated runway and unlock themselves from a board while being dunked under water.

Las Vegas Show

Contestants must race to milk a goat with their mouths, and unlock themselves from a box which let's them fall from a helicopter.

Miss USA Fear Factor

Contestants must drop out of a helicopter and race to pick up flags while swimming, and participate in a gross pot luck dinner. Then they must make their way through an airborne greasy maze.

Semi Finals: Part 1

Male contestants must identify disgusting items, then eat any they guess incorrectly; female contestants must retrieve dead feeder mice with their mouths while swimming in a vat of dead rodents.

Semi Finals: Part 2

Contestants must retrieve items from the bottom of a tank filled with fish scales and surf tanker truck as they are bucked off by swerving drivers.

Contestants will be seated in the driver's seat and truck of a stretch limousine suspended on a 45-degree angle just above the surface of the water. Contestants will have to transfer a gross blend of bugs through a plexiglass tube. Contestants are tied into their seat and to the steering wheel of a convertible automobile, which will be hanging over a cliff approx 100-feet in the air.

Contestants will be laying on their back bound at their ankles, waist, and one wrist to a plexiglass platform that is hung from a helicopter. Contestants will play a game to determine how much skunk, possum, rattlesnake and armadillo they will have to eat. Contestants will have to cross 4, 20-foot rotating beams placing flags along the way.

Contestants will be locked in a plexiglass box that will be hung from a crane and lowered into the water leaving approx. 1-foot above the water for breathing. The other contestant will be outside of the box where they will build an unlocking device and free their partner. A contestant bobs into a tank of gelatinous fluid for animal tongues. The final team stunt includes a truck car ramp.

Previous winners return to ride bicycles on a 6-inch-wide beam balanced between two speeding trucks and drink juice made from tomato horn worms.

Contestants must free themselves from diving helmets filled with moray eels, then cross from one pontoon to another on a seaplane flying thousands of feet in the air.

The contestants will be bound face to face by their wrists to a pole that is attached to a circular platform hanging under a helicopter. Two contestants are placed into a single plexiglass helmet filled with thousands of spiders. The contestants will be bound by their ankles to a platform which is attached to a lever that is weighted at the opposite end holding the platform in the air.

Contestants will be strapped by their waist, ankles, and one wrist and locked to the face of a 20-foot diameter wheel that is set halfway into the water. Contestants drink various types of disgusting liquids at the Fear Factor Bar. Contestants must stand on a swaying platform that is approx. 3' by 3' hanging beneath a helicopter.

Contestants walk across a metal beam while being zapped with electricity and ride through a bug storm.

Contestants must grab a flag from the bottom of the Roosevelt Island tram as it traverses the East River, dine on rat stew served up in Times Square and perform an aerial stunt dangling from helicopters near the Statue of Liberty.

In this couples stunt, the contestants will be standing on opposite platforms suspended 100 feet in the air. Contestants will have to siphon gross fluid (worms, crickets, cow parts, pig parts etc) out of 4 containers and into a large funnel.

Contestants must snag flags from poles attached to the front of a moving dump truck and a spinning car suspended over a lake.

Contestants will be standing on top of a plexiglass platform that covers the surface area of a water tank. On "go", the first team member must enter the water through a vertical tube. Contestants will be standing on a tube that is suspended approx. 100' in the air from a crane. The tube will begin to roll and the contestants must stay balanced on it.

Contestants must choose three of five taste treats from a menu of sheep brain pie, bile gelatin with cave spider topping, night crawler green beans, worm and beetle stuffing and maggoty mashed potatoes.

Contestants attempt to deliver presents despite the presence of three vicious guard dogs and try to mow down snowmen with cars, flipped at high speed.

Contestants swing between speeding trucks and dine on special stuffed doughnuts.

Seven couples remain in the competition; contestants are yanked from the top of a 12-story building; one contestant on a raft in a smelly swamp must catch objects thrown by his or her partner.

Contestants launch a piece of cow stomach to their partner on a small raft in the middle of a swamp. In this couples stunt, the contestants will be standing at a podium on the roof of a 5 story building. On "go" the couples much each complete an eating task. Once they have completed the above task they will be yanked off of the roof of the building and dangled from a crane.

Couples take a drive with the driver blindfolded, guided only by the voice of the partner, aiming to chase down a flatbed truck and drive on top of it.

Contestants guide their partners through an electronically charged maze filled with trip wires that deliver a jolt.

The male contestants are trapped in a cage filled with huge bullfrogs as their partners scramble through pits of rats and snakes, looking for the keys to free them.

In this final couples stunt, both contestants will be riding on the back of an Ultraboat they each must grab hold of their own rope ladder hanging from a helicopter. Once touching down, contestants enter a trailer bed of a semi trailer and complete a small task to retrieve a car key, hop into a car, fasten their seatbelts and drive the car out of the back of the semi.

In this stunt a rope will be suspended between two helicopters. On "go" the contestant must traverse the rope from beneath one helicopter to the other end of the rope beneath the other helicopter while releasing flags along the way. For the last stunt, the contestants will be driving their stunt car over a ramp car.

Contestants must grab a flag from a half-submerged car on an underwater rotisserie.

For this twins stunt, one contestant will be seated in a helicopter and the other will be on a quick-release. On "go" the helicopter will fly towards a drop zone where the seated contestant will release their twin to the water. Working as a team, couples will tear chunks of meat off of a cow jaw using only their mouths and run through a 30ft trench filled with cow parts.

In this couples stunt, two tightropes and two hand lines will be strung parallel of each other from windows of the Mandalay Bay Hotel. On each tightrope, small sandbags will be tied. On "go" each teammate must untie and toss their sandbag to their teammate on the opposite rope.

Newly married couples eat a "Fear Factor" wedding cake complete with alligator eyeballs and free themselves from a sinking vehicle.

Contestants will climb through a 30ft plexiglass tunnel while a fire hose sprays them with a gross fluid. A truss structure will be wrapped on all four sides and bottom by cargo netting and hung beneath a helicopter. On "go" the contestant will begin to work their way around the outside of the cargo netting to release flags along the way.

Four pairs of best friends play a game of "Fear Factor chicken" in which contestants drive cars toward each other, only to have them flipped before they can collide.

Contestants will be lying on their backs in a muddy trough on the roof of a building. On "go" the contestants will be dragged through the mud and must pluck as many flags as they can before dropping off the edge of the building.

Contestants take part in death-defying, stomach-churning stunts. This time, it's a helicopter net climb from a fireboat, shots of habanero followed by throwing flares, and a 60-foot fire slide into an ice-covered pool.

Contestants take part in death-defying, stomach-churning stunts. This time, it's jumping between two suspended platforms, a spin at barbershop roulette, and an explosive container launch.

Classic Fear Factor

Contestants take part in death-defying, stomach-churning stunts. This time, it's exploring a pig pen with their mouths, seeing how it feels to become a rotisserie chicken and a rotting milk escape attempt.
